Please arrive 30 minutes before your appointment to register and complete a health questionnaire.
When preparing for your appointment, please remember to bring:
Driver's license or state-issued ID
Digital copies of x-rays, MRIs, and other imaging, as well as written reports from the radiologist
Health insurance information
List of medications, if any
Records from any previous surgeries performed elsewhere
It is not uncommon for us to require additional imaging for evaluation and treatments.
